Name, Address, & Phone will be included.

If you have information that you do not want included in the Buzz Book - fill out the form
Select - Check Comment section - put in comment section which data not to publish.  

If you DO NOT want to be included - Select  - NO - DO NOT include my students information

The Buzz Book will be available at the end of October.
